Connecticut CAA's Customer Intake and Assessment System
and Outcome Matrix

In order to determine the status or baseline of each customer or family participating in a Connecticut CAA case-management program or individual program or services, an initial universal intake and pre-assessment will be conducted on each and every customer. The initial universal intake and pre-assessment are a series of questions designed to identify immediate needs and to determine whether the person has multiple needs and could best benefit from case management.

The Outcome Scale Matrix is grouped by 17 dimensions that are the building blocks of family stability and self-sufficiency. Each of the 17 dimensions has its associated assessment questions and matched outcome scales. Taken together, the outcome scales form an outcome matrix that provides an overall assessment of a family’s movement towards self-sufficiency or stability.

Responses to the groupings of assessment questions are used to create a score on each of the outcome scales and an overall score on the outcome matrix. Responses to individual questions can also be analyzed independent of their use in the scales. The initial assessment is used to create a baseline for each client or family. Subsequent assessments measure the customers progress toward self-sufficiency. The Outcome Scale is also linked together with the Outcome Catalog to create a comprehensive reporting and tracking system.
